Wound Care Documentation

Program Summary

   The documentation captured, during an encounter, becomes part of the patient’s legal medical record. As healthcare providers, we believe one of the medical record’s most critical functions is to plan and provide continuity of care for a patient’s medical treatment.  Healthcare providers need to also remember the additional functions of the medical record, including providing information for the financial reimbursement, information for quality improvement/assurance and peer-review committees, state licensing and state regulatory agencies when assessing the quality of care provided and the critical information in an accreditation process.

   Common documentation and coverage issues will often result in denials. Learn how to safe guard your agency against documentation issues. In this interactive program, we will discuss the different types of wounds, the new OASIS changes with OASIS C2 related to the wound section, and continuing documentation of medical necessity for coverage. 

Objectives:

  • Explain the phases of wound healing
  • Identify the four stages of pressure ulcers
  • Identify characteristics of lower extremity ulcers
  • Identify appropriate documentation and treatment for ulcers 

Griffin Faculty:  J’non Griffin, RN, MHA, WCC, HCS-D, COS-C, is president of  Home Health Solutions, LLC and a nationally recognized speaker with over  26 years of home health experience. She has experience as a field nurse,  supervisor, quality assurance director, staff development and appeals  coordinator, and chief operations officer. Her responsibilities have included  risk management, acting as compliance officer, development of policies and  forms, survey compliance, appeals of Medicare denials, writing Corrective  Action Plans, start-ups of parent and branch agencies, and consulting with  agencies on a variety of subjects and education of staff.
