Using the Guide

To make the Referral Guide more intuitive, agencies are grouped into Home Care and Hospice sections. If an agency provides both home care and hospice services, they will appear in both sections. A third section spotlights Affiliate Members that provide essential services to the industry.

Home Care and Hospice locations are assigned to one of six Ohio regions based on their address. If an agency has more than one Ohio location, each will appear in the relevant region. A statewide Ohio region map is included on page 17. OCHCH follows a county-based, six-region arrangement in line with Ohio’s economic development agencies.

Please refer to a specific agency’s website for details on their service area, accreditation, licensing, certification, accepted payer sources, and other details. Information presented in this guide is believed to be accurate at the time of publication. Contact us any time at ochch@ochch.org to suggest an update.
