Provider Membership
Provider membership is open to any business that provides home health, hospice, case management, therapy, homemaker, and/or home health aide services in Ohio. Annual dues for Provider members are based on gross revenue generated in Ohio from all locations and all patient revenue payers.

What’s an Education Pass?
Private Duty Membership
Private Duty membership is available to agencies whose primary services include personal care, companionship, and assistance with daily living activities. Unlike traditional home health or hospice agencies, Private Duty agencies operate independently of state or federal government funding. They primarily serve clients who pay out-of-pocket or utilize private insurance.
This is a “voting membership” with each provider member eligible to cast one vote on important decisions and leadership roles of the association.

Affiliate membership is open to businesses and organizations that provide products and services to health care at home providers.

The OCHCH Education Pass membership upgrade enables no-cost or deeply discounted registration to a wide range of OCHCH-managed and select partner-managed educational opportunities posted on our Education Calendar.
Smaller agencies will save $1,000 vs. 2025 pricing! Mid-size and Large agencies save $500! No more codes or limits on premium education such as OASIS Two-Day Training! Read the full announcement here.
Realizing the savings is even easier this year. One registration to the Two-Day OASIS Training would normally cost $499 without an Education Pass. For smaller agencies, sending even one team member will cover your Education Pass cost!
Education Pass price based on Annual Gross Revenue

Important Details
- An active OCHCH Provider Membership is required to utilize the Education Pass.
- Home Health Aide Live Training Sessions are not included (see the HHA Training Pass section).
- New Members – to apply for membership and an Education Pass at once, use the “Provider Membership with Education Pass” option on this page (opening 12/16).
- Current Members – to add an Education Pass contact us for an invoice (self-serve online purchase coming soon).
- Event Registrations at Education Pass pricing require an active Pass at the time of the event. If a Pass has lapsed or been deactivated prior to the start of the event, the attendee must re-register at the appropriate rate or reactivate their Pass to retain Pass pricing.
- All Passes are valid for 1 year from date of purchase.
- Education Passes are not transferrable to other agencies and may not be used to register individuals not employed by the member agency.

Home Health Aide Training Program Pass
Are you an agency owner/operator who wants to train your staff to meet the requirements for a home health aide? Do you have many aides to train? The Home Health Aide Training Program Pass is an annual cost-savings program that provides you with the tools and resources your agency needs to efficiently train home health aides at scale.
Level 1: $2,000
- Ten (10) trainee registrations per year included in the price of the pass, a $2,490 value.
- Each additional trainee may register at the discounted price of $125 each.
- Saves $49 each vs. the regular member rate on your first 10 trainees, then $124 each afterwards.
- Well suited for agencies planning to train more than eight aides per year.
Level 2: $3,000
- 20 Home Health Aide trainee registrations included, a $4,980 value.
- Each additional trainee may register at the discounted price of $100 each.
- Saves $99 each vs. the regular member rate on your first 20 trainees, then $149 each afterwards.
- Ideal for higher volume agencies.

- An active OCHCH Provider Membership is required to utilize the Home Health Aide Pass. To apply for membership and a Pass in one convenient form, please use the Provider Membership form above. For active members that wish to add a Pass to their account, use the link in this row.
- Registrations at Pass pricing require an active Pass at the time of the event. If a Pass has lapsed or been deactivated prior to the start of the event, the attendee must re-register at the appropriate rate or reactivate their Pass to retain Pass pricing.
- Training Passes are not transferrable to other agencies and may not be used to register individuals not employed by the member agency.
- Pass allocations are valid for one year from purchase and unused registrations are not refundable.
Educational Institution Membership
OCHCH offers institutions of higher learning the opportunity to join a network dedicated to expanding education, training, and exposure to home and community-based services. Membership benefits include access to unique value-added resources, fostering the next generation of healthcare professionals. Membership for post-secondary schools is based on the type of institution: State or Private University, Community College, Career Center, or For-Profit College.
